Room C, the toddler classroom did not have the mats/cots labeled within the classroom. A total of 12 children were observed on mats that were not labeled, and/or labeled incorrectly. The operation will utilize a number chart to specify which cot/mat belongs to which child. The numbers were added to cots while Licensing was present.
The annual sanitation inspection report was dated 05/09/2022.
A caregiver in the school-age classroom was observed caring for seven children, 7-9 years of age. The caregiver observed was not following the provisional status, which states that the individual could not care for children alone, or unsupervised. One caregiver was added to the classroom in order to abide by the provisional status.
Based on the information obtained, the transportation driver did not have a list of children who was being transported. One transporter was observed unloading 3 children from the transportation vehicle that picks up from Mueller Elementary.
Four out of ten staff records observed did not include a total of 24 annual hours. Half of the training hours observed were provided by individuals who do not qualify. Staff files did not include topics related to: - Emergency preparedness - Growth and development - SIDS/shaken baby syndrome - Abuse and neglect - communicable diseases - administering medication - food allergies - building and physical premises safety - handling, storing, and disposing of hazardous materials
One caregiver in the infant classroom was observed diaper changing an infant, 10 months of age. The caregiver left the classroom without washing their hands, and/or the infant classroom. After being notified the caregiver washed the infant's hand.
The caregivers in the infant classroom was not aware of the ages of the youngest and the oldest in care. The caregivers in the 2 year old classroom was not aware of the youngest and oldest child in care.
Based on the documentation received, the transportation training certificate observed for the driver in question did not meet minimum standards trainer's qualifications.
Based upon the information obtained during the investigation, a caregiver failed to follow proper procedures to ensure all children they were responsible for were accounted for. The caregiver called a child's name, another child responded the child was present, however, the caregiver failed to conduct a name-to-face to verify the child was present. Approximately 20-30 minutes later, it was discovered the child was not in the building due to being left on the center's vehicle.
Based upon the information obtained during the investigation, verification was not available for review to support the facility completed and shared an incident report with the parent and/or guardian of a child that left unattended on the center's vehicle.
Based off the information obtained from the investigation the operation failed to report that a child was left on the transportation vehicle unsupervised for an extended period of time.
Based on the documentation received, the transportation training certificate observed for the driver in question did not meet minimum standards trainer's qualifications.

Based upon the information gathered during the investigation, evidence was available to support the safety precautions for unloading children was not followed. As a result, a child was left on the van unattended for approximately 20-30 minutes.
Based upon the information obtained during the investigation, sufficient evidence was available to support, the driver failed to follow the facilities procedures regarding safety precautions for loading and unloading children which resulted in a child being left in the center's vehicle unattended.
The operation had not completed a Childrens Product Safety Annual Certification since 1/13/2020. *Note - This was corrected during the inspection when the Annual Certification was completed.
The first aid kits in both of the transportation vehicles was missing antiseptic. *Note - This was corrected during the inspection when antiseptic was added to both first aid kits.

The operational policies did not have a policy for safe sleep for infants. *Note - This was corrected during the inspection when this policy was added to the operational policies.
Six infant bottles were not properly labeled with the infant's first name and last initial. *Note - This was corrected during the inspection when the bottles were properly labeled.
A caregiver was allowed to have direct access to the children with an inactivated background check. *Note - This was corrected during the inspection when the caregiver was sent home until the results of a new background check is received.
