There was sufficient evidence obtained from a DFPS investigation to conclude that operation staff failed to provide the level of supervision/proximity necessary to ensure a child’s safety as witnesses confirmed that a child was able to exit the classroom and front door of the building, resulting in the child gaining access to the parking lot and briefly leaving the premises prior to being apprehended by operation staff.
