Some examples include: children did not wear any identifying information with the operation's name and phone number such as a shirt or nametag; permission slips were not always signed; there was no 48-hour notice posted; emergency consents or contact information was not taken.
The operation was out of ratio when they go on walks outside of the daycare in the neighborhood. There were at least 12 children present ranging in age from 18 months to 4 years old with only 3-4 caregivers present.
