In the nursery 1 classroom the ceiling tile was missing and was exposing insulation due to water leak. Water was leaking from ceiling and there were three buckets of water on the floor collecting water. This classroom was not being used at the time inspection. The drain in the floor in Nursery 2 restroom had broken concrete and tile which posed a tripping hazard. This was corrected at inspection when a plastic cover was put over the drain.
Test data from the manufacturer was not available for review regarding the unitary material (turf) on the playground at the time of inspection.
While observing diapering procedures in the 2-year-old room, the caregiver did not sanitize the diapering mat after changing a child's diaper and before placing another child.
Bleach bottle was observed on the sink in Nursery 2 classroom and was accessible to children. This was corrected when the bleach was put away out of children's reach.
