Based on the information obtained during the investigation, sufficient evidence was available to support the director allowed a newly hired employee to have unsupervised access to children without first completing the required 8-hrs of Pre-Service Training nor had the individual completed an abuse/neglect training.

Based upon the information obtained during the investigation, sufficient evidence was available to support, the driver who transported a group of children, failed to follow the facilities procedures regarding safety precautions for loading and unloading children which resulted in a child being left in the center's vehicle unattended.
Documentation was not available upon request to validate the director and any caregiver who transports completed the required 2 hours of Transportation Safety Training.
Documentation of the current fire inspection was not available for review during the inspection. The last documented fire inspection available to licensing was conducted on 09/24/2020.
Two caregivers were observed to be in charge of a total of 14 children. When asked how many children they had, 1 caregiver stated they were responsible for 9 children, the other caregiver stated they were responsible for 7 children. When asked again, the caregivers were not able to provide an accurate number of children they were responsible for. CCR informed the caregivers there were only 14 children, then it was stated, two children just went home.
Certification was not available for reviiew to support the facility did not have any items recalled by the Consumer Product Safety Commission.
