A child was left in the classroom for less than five minutes as the rest of the children were doing their outside time.

Observed five children sippy cups that were not labeled with each child's name. Note: All training cups were labeled with each child's first and last name.

A caregiver in a classroom (age 3y-4y) and a caregiver in a different classroom (age 4y-6y) did not know the correct number of children that were currently present in their classrooms during the inspection.

Two children records evaluated were missing the special needs acknowledgment from their admission forms.

The operation has not practiced a lock down drill at least four times a year. The last lock down drill was conducted on 9/28/2020.

The operation is missing the name and address of the alternate shelter away from the center from the emergency preparedness plan. The operation is also missing how children will be evacuated or relocated to the designated safe area or alternate shelter, including specific procedures for evacuating and relocating children who are under 24 months of age, who have limited mobility, or who otherwise may need assistance in an emergency, such as children who have mental, visual, or hearing impairments. Note: The Director added the name and address of the alternate shelter away from the center and how children will be evacuated or relocated to the designated safe area or alternate shelter ho under the age of 24 months or with limited mobility to the emergency preparedness plan during the inspection.
