A child was obserevd unattended in a dark classroom.

1 out of 6 staff files reviewed is not current with First Aid and CPR.

The caregiver failed to wash the child's hands following a diaper change.

A caregiver hired on 10/29/24 and counted in ratio did not have 8 hours of pre-service training.

There was no documentation showing how a caregiver meets the education requirements.

There was one caregiver caring for 15 children. The specified age for the group was 2 years which allows 11 children to 1 caregiver. Note The ratios were left in compliance.

Two personnel records did not have the following information: Affidavit, education translation, and statement of orientation.

The outdoor play area was observed with wooden logs with sharp edges and rough surface areas prone for splinters. There was multiple small paper and plastic pieces scattered throughout the mulch in the outdoor area used by the children. There were overgrown weeds in the outdoor area along the wall.

Director did not have the required annual training topics for training period, 8/01/22-8/01/23.

The operation does not have a current Sanitation Inspection. The last health inspection conducted was on 6/24/22.

Director did not have the following trainings for the training period, 8/2021-8/2022: (1)Recognizing and preventing shaken baby syndrome and abusive head trauma (2)Understanding and using safe sleep practices and preventing sudden infant death syndrome (SIDS) and (3)Understanding early childhood brain development. -- Transportation training

There was only three out of four lock down drills conducted for 2021.

There was no documentation for an employee hired on August 2021 for an Orientation and Pre-Service Training.

A child with a diagnosed food allergy did not have a Food Allergy Emergency Plan.

A climbing play structure did not have the proper depth of surfacing/mulch.
