One tire swing structure that did not have the required use zone. The operation will not use this structure until the required use zone can be obtained.

Out of 6 children records reviewed, one child was missing an immunization record.

A slide structure was observed without sufficient loose fill. Four individual swing structures were observed without loose fill. The operation will not use these structures until the required loose fill is acquired.

Out of 6 staff records reviewed, 2 staff did not have the required infant pre-service training.

Out of 6 staff records reviewed, 2 staff did not have proof of all the required training topics and 2 did not have proof of the required orientation in these topics.

It was determined that the operation had knives in the kitchen's dish rack that was accessible to children in care. This was corrected during the sanitation inspection when the knives were put away per the APH inspector request.

Based on observation and the evidence obtained throughout the investigation, it was determined that the AC unit is accessible to children. The fence securing the AC unit has a 5-inch gap in-between the gate and the fence. The gate is without a lock and can be easily opened.

The operation's emergency preparedness plan needs to be updated to include secondary location, the steps for transporting to the location, a reunification plan, and details for all types of emergency situations.
