One caregiver responsible for toddler aged children stated that they had 19 children present in the classroom. There were 20 children present and counted during inspection.
A diaper changing mat was observed to be ripped and torn, preventing the surface form being properly cleaned and sanitized between diaper changes. The damaged mat was removed during inspection and replaced with a new mat.
There was not a complete first aid kit available for review during the inspection.
There was sufficient evidence found during the investigation to show that good judgement was not used when a caregiver was allowed to work when they were ill and with fever.
One staff member did not have an eligible background check during the inspection. The staff member was asked to leave the operation.
It was observed by video camera recording that a child was left alone without supervision in a classroom for less than 5 minutes.
