Based on information obtained during the investigation, it was determined that while the operation did self-report a child's injury to Licensing, they failed to make that report within the required (2 day) timeframe. The injury occurred on 4/5/22 & the report was received by Licensing on 5/3/22. The child's injury required medical treatment.
