A caregiver failed to intervene when two four-year-old children were fighting resulting in a eight-year-old child in attempting to stop the fight by grabbing a four year old child around the neck and pulling her back.

The most recent fire inspection was conducted on 5/12/2023.

A grate in the playground is accessible to children. The sandbox needs to be closed when not in use. A piece of play equipment must be placed over surfacing before being used by children in care.

Three of eighteen files reviewed were missing information. One file is missing the date of admission, Two files for four year old children who are not yet attending school are missing hearing and vision screening.

Two transportation vehicle have not been maintained. 1 transportation vehicle's seat belt is not working properly and the entrance double door does not open. 1 transportation vehicle's back door does not open.

7 employees no longer employed more than 7 days have not been inactivated at the time of inspection.

Based on documents reviewed information on sign in/out log is incomplete.

Based on the information obtained standard was found non-compliance. Out of the 2 staff files reviewed, Both staff did not have remaining pre-service within the 90 days of employment. 1 staff is lacking the remaining 16 hours of pre-service training needed. 1 staff is lacking the remaining 8 hours of pre-service training needed.
