At time of inspection, one caregiver did not have the CPR certification completed in person.
At time of inspection a toddler caregiver stated to have 13 children when the class had 16. At time of inspection a toddler caregiver was not able to provide the age of the oldest in class.
During the course of the investigation, it was determined that a child received a less serious injury and the parent was not notified when the child was picked up from the child-care center.
During an investigation, it was determined by observations and interviews that caregivers were not wiping the mucus from children's noses.
Based on the DFPS investigation, the operation failed to notify the parent at pick up that their seven-month-old infant bumped their head on a shelf, which resulted in a red mark on the forehead. The red mark turned into a bruise.
The changing mat in the infant room was observed to be torn in several places. Note: This was CORRECTED AT INSPECTION when the changing mat was replaced.
