The operation had more than 12 children in care. *3 children ages 7m, 9m and 18m remained in the infant room with 1 caregiver. All other children were moved to the pre-k room with 2 caregivers*

There was a loose section of the bottom moulding trim in the infant room which created a small opening on the wall. There were water stains on the ceiling on one of the classrooms used by the children.
There was one caregiver caring for 5 infants. The specified age for the group was 0-11 months which allows 4 infants to 1 caregiver. Note The ratios were left in compliance as an additional caregiver was put in the classroom.
Training certificates were not available for review.
Two translated education documents for two caregivers did not indicate the equivalency of education in the U.S.A.
There was no Food Allergy Emergency Plan for a child diagnosed with a food allergy.
There was trashcan without a lid in a classroom and the trash was accessible to the children in care. There were 4 tricycles with chipped paint and other bicycles with missing the rubber handle grips. There were purses accessible to the children in the classroom. Note: Purses were moved out of reach of the children.
Five caregivers did not have a current CPR/First Aid training.
