A stationary play car did not have the required six feet of use zone around it, there was a play house right next to it.

The director is missing 27 hours of training for training year 06/3/2025 to 06/3/2026. The operation provided 18 hours for training year 6/2/2026 to 6/3/2027. The director is missing 9 hours.

One child had expired medication from 8/2023, this medication was not given back to parents or discard.

Two children with medication was not in the original container, an inhaler and EpiPen.

Four out of six employees did not have available to review the complete hours of the annual trainings.

The sanitation inspection report was not available to review.

The Pre-k playground does not have proper depth of mulch maintained throughout the area. This could cause an injury if a fall occurs on playground.

During naptime 1 caregiver was supervising 10 infants and children ages 12-22 months. Additionally 1 caregiver was supervising 42 children ages 3-5 years old with a specified age of 3, putting the room over naptime ratio by 7.

Two caregivers were present around children prior to receiving notices of eligibilty.
