There was a missing immunization report for one out of four files.

The director has an expired CPR card on file.

The shelf in the pre-k room is peeling and paint chipping. The chairs in the school age room has the wood peeling and chipping. The couch in the toddler room is ripped. Note: The items were moved out of the classrooms bring this into compliance.

The CPSC posted on the parent board has not been completed since 2017. Note: The current certificate was found by the director in her office.

Three of the five children files reviewed have the emergency contact as their parent.

A wooden chair in the toddler room was observed to be broken causing the seat and back rest to have sharp edges. Note: This chair was removed from the classroom and thrown out to the trash.

One child's file out of 4 reviewed did not have a current immunization record.

I observed a rope on the fence on the play ground in which children had access to in 2 diferent places, The director removed the ropes from the fence and this was corrected at inspection. A photos were taken.

I observed 2 infants placed in a high chair table without any safety straps. The director replaced the safety strapped in the high chair table.

The director does not have the required annual training topics for the last annual training year.
