During the inspection the loose-fill was observed under the required amount and with grass growing around it.

Through an investigation inspection it was determined that caregivers were not disciplining their own children still in care as they would other children in their care.

Through an investigation it was determined that multiple caregiver threatened corporal punishment.

During the insepction areas of the walls and air grates in reach of children were not kept clean.

During the investigation inspection a child was observed unsupervised in the daycare with the caregiver unaware of their location.

During the inspection a mop bucket was observed in the hallway in front of the infant rooms full of water. The bucket was removed during the inspection.

A class was observed to clean up and sat down to eat a meal without handwashing, the tables being sanitized or the teacher washing hands as she served.

During the investigation it was found that the infant class and the older toddler/two class mix on the playground and one class is not going inside when the other class comes out.

During the investigation, diapering areas as well as children's tables were observed to be dirty and need to be sanitized.

The operation had open paint cans, large painting equipment, tools, and other chemicals including a fire extinguisher where children could access them. Chemicals and tools were observed in the kitchen food prep area.

During the investigation, the infant room was being renovated and an infant was placed in a room with children more than 18 months older. During inspection, the room was cleaned up and the infant was taken to the infant room.

During inspection the plastic borders that enclose the loose fill was observed to be cracked and broken.

The parent handbook was reviewed and was missing the following procedures: 1) How to view standards online, to report abuse/neglect, and the HHSC website 2)A list of the require parents rights 3) how the operation will support children with special needs.

The emergency preparedness plan in the parent handbook does not include the required information and needs more detail.

During inspection a section of the wall has peeling paint and needs repair.

The loose fill was observed to be compacted and needs to be either removed and replaced or additional inches added to proper depth. Compacted earth cannot be included in the inches.
