During the investigation it was found a caregiver used prohibited discipline by grabbing on a child, picking the child up by one arm, and placing the child in a seated position roughly on the floor.

During the investigation inspection, it was found that the caregiver did not use good judgement or self-control when the caregiver tapped the child on the head with a block.

There were two buckets that had standing water and planting soil in them. This was corrected when it was removed from the playground.

During the investigation inspection it was observed that an infant was sleeping on a boppy, on the floor, and was not placed in their crib after falling asleep.

During the investigation it was found that the operation's staff was not consistently intervening in situations that needed intervention in order to keep each child safe.

During the investigation it was found that there was forms of prohibited punishment being used at the operation.
