One of the staff members was still active after their last days of work was February 27, 2026. NOTE: This was corrected at inspection when they were made inactive.
On the school age playground, a picnic tabletop was observed with jagged edges and broken. On the toddler playground a child size round tabletop table was observed lifted and warped. NOTE: Compliance has been met when the round tabletop table was removed.

It is determined through investigation evidence a caregiver yelled at and used ridiculing language to speak to children in care.
It is found during an investigation that a caregiver grabbed a child under their arms in an effort to get them to clean up resulting in a whelp under their arm.
Caregivers did not know how many children were in their group while transitioning back to their classroom, which resulted in a child being left on the playground.
Supervision was compromised when a child was left alone on the playground.
During the course of an investigation conducted by DFPS, it was determined that a child was neglectfully supervised by two caregivers when left on the playground for 17 minutes. Caregivers were unaware of the child's absence during this time.
