A child under 2 years old was observed in a classroom were a television was observed on showing cartoons.

It was observed that 2 out of the 6 caregivers did not have an up to date First Aid/CPR training.

Operational policiess reviewed, written operational policies was missing the section for promotion of physicical indoor and outdoor activity policy.

2 out of the 6 records reviewed were missing immunization records,

3 toodlers and one infant observed in high chair not strapped.

The recent daily buidling ground checklist was not available for review. A glass was observed in the outdoor area, was picked up and handed to director to throw away. last daily biuilding ground check was done 10/18/2021.

Fire extinguisher in operation and in transportation van dated July 2020.

Current CPR training was not avialble for review.

An employee records has 19 hours of annual training hours required.
