The operation is out of compliance with child/staff ratio when combining the two and three years olds into one classroom.

The metal pins used for anchoring the pieces of climbing equipment on the playground are sticking up from the ground. This could cause a tripping hazards or injure a child if they were to fall on it.

Evidence gathered during this investigation show two caregivers were not using good judgment in the way they were handling children during nap time.

Evidence gathered during this investigation shows caregivers were on their cell phones while in direct care of children.

During the course of the DFPS investigation it was determined day care staff are vaping on day care property while children are in care.

During the course of the DFPS investigation it was determined that staff are using prohibited discipline on their own children while at the operation. Day care staff have to follow the same discipline policy for all children in care including staff's own children while at the operation.

Staff did not wash their own hands or children's hands after each diaper change.

Staff are not sanitizing the diaper changing mat after each diaper change.
