Based on the information obtained during the investigation, there is sufficient evidence to verify that caregivers did not maintain visual awareness of each child’s ongoing activities. Caregivers did not intervene when children received unexplained injuries on various occasions.

The operation had not submitted the total number of employees who ceased working at the operation the previous calendar year by January 15, 2024.
