It was determined through the course of the investigation that a caregiver did not meet the minimum education requirements.

Portions of the wood flooring are detached from the floor and loose causing a tripping hazard. Portions of the wall are chipped in the restroom. There is a hole in the wood floor near the back entrance of the operation that is used to enter and exit to the playground.
An employee disciplined their own child, however the discipline included spanking which violated the requirements in the discipline subchapter.
