Based on information obtained during the course of the investigation, the operation failed to contact EMS when a child required immediate medical attention.
In the pre-k classroom, a hole was observed in the wall and the paint was observed to be chipped and peeling.

In the other class one caregiver was observed supervising 7 children ranging from ages 3 months to 15 months. Specified age group was 12-17. Four infants were observed to be sitting in a toddler table for over 30 minutes. Three infants were observed to be in bouncer seats for over 30 minutes. Note: It was corrected during inspection.

The operation had a climb and crawl catepillar on concrete. It did not have the required surfacing. Step 2 structure with a slide of 27 inches in height did not have the required unitary surfacing.
Eight employee files were reviewed. One employee hired 3 months ago did not have the pre-service training. Two employees were pending training in the following topics: administering medication;preventing and controlling the spread of communicable diseases, including immunizations; understanding building and physical premises safety; and handling, storing, and disposing of hazardous materials .
Emergency medical transport and treatment authorization forms for each child being transported were not available for review.
Children's products certification form was last completed on November 25th, 2020.
