There was a toddler slide that had a barrier wall. On the other side of wall, it had a broken piece of plastic exposing a sharp edge. Note. This was corrected at inspection when the director and I removed the slide from the play area and out of license space.

During the course of the investigation, it was determined that at least one child was left out in the hallway unsupervised for approximately a minute.

Recite: In a room with a specified age of 0-11 months, a caregiver was alone with six infants. In a room with children ranging in age from 18-24 months, the caregiver was alone with 10 children. Noted: This was corrected when another caregiver went to the infant room and when a child from the 18-24 months room was transitioned to another room. All rooms were then in ratio.
Three infants four - six months were observed to be drinking a bottle in a bouncer or in their play yard, not being held by a caregiver.
An infant was observed to have their bottle propped up to their mouth with a blanket.
There were no time sheets or other documentation showing the director's attendance during operating hours.
An individual, who was being counted in ratio, did not have a background check submitted. Noted: The individual left the operation while I was present.
An infant was observed to be using a walker. This was corrected when the caregiver removed the infant from the walker.
Recite: The older infant classroom had one caregiver with 8 children. The caregiver was over ratio by 3 with the specified age group of 12-17 months. This was corrected at inspection when another caregiver was added into ratio.
Electrical outlet was accessible to a child younger than five years without a childproof cover. This was corrected when caregiver added childproof covers to outlets.
Children in Room 3 were observed eating directly from the table. This was corrected when the caregiver placed the food on plates.
2 infants were on their chairs being fed by caregiver however the safety straps were not fastened. This was corrected when the caregiver fastened both of the infant's safety straps.
There are alot of small red ants in the older infant room.
There were food crumbs, harden food and dirt on the older infant's room between the ABC mats and under the mats.
Two individuals were present in the center and were included in ratio, which meant there were left alone with the children despite not having complete background checks.
Infant classroom was one infant over ratio with the specified age of 0-11 months. The older infant classroom was over ratio by 2 with the specified age group of 12-17 months. The toddler room was over ratio by 3 with a specified age group of 2 year olds. This was corrected at inspection, when 2 staff members were counted in ratio and 3 children were sent home to come in compliance with ratios.
Two older infants were observed sitting on two broken chairs. These chairs are hook on high chairs that attached to the table. This was corrected when the children were removed from the chairs and placed on other chairs that were not broken. The broken chairs were set aside.
The infant classroom was observed out of ratio during naptime. There was a total of 6 infants with a specified age group of 12-17 months, with only one caregiver. This was corrected when a second caregiver joined the room.
A child had a seizure at the operation and was take to the hospital by EMS. The operation did not notify CCR about the incident within 48 hours.
