The door leading to the playground from the pre-k classroom is missing a doorknob. There is broken board on a pretend play structure in the playground.
Based on the investigation it was found that an employee at the operation grabbed a child and took him to the office when he did not want to go and was struggling against this.
A child was not supervised properly and child was able to exit the operation through a gate. Child was able to get to the front of the daycare building unattended.
A caregiver did not use good judgement when they lied to law enforcement.
