Based on the information obtained, it was determined that a child was unknowingly left outside of the CDC’s playground door while his class transitioned back to their classroom. This was corrected when an elementary school teacher, who was on the older children playground, found the child and assisted him back to his caregivers.

Based on the information obtained, it was determined that a child was left unattended in the classroom for roughly 30 minutes. The shool corrected this by providing additional training to all caregivers and using a face to name chart during each transitioning period.
