Caregiver did not demonstrate good judgement when she picked up child by the arm. It resulted in the child to sustain a mild bowing of the left radius in the forearm.

Three out of three employee records monitored do not have the annual training required for hazardous material, building safety, and communicable diseases.

Of the two Admission Forms reviewed, one did not have the Date of Admission recorded for the student. This was corrected during the inspection when brought to the director attention. The director recorded the date of admission on the Admission form.

Loose fill in the area of the play scape is not at the proper depth of 6 inches.

Of the four children files reviewed, two did not have the Date of Admission written on the Admission form. This was corrected during the Inspection when the Director wrote the Admission Date on the Admission Form.

Observed two missing tiles on the kitchen ceiling. Water could be heard and was observed leaking from the ceiling. The missing tiles pose a hazard to the children and caregivers.
