Infant classroom was observed to have one caregiver with 5 infants a specified age of 0-11 months.
Classrooms were observed to be out of ratio. According to sign in and out logs classrooms are out of ratio routinely througout the day.
Operation staff did not show good judgement by incorrectly informing a parent of an incident.
A child was left outside for 12 minutes unattended.
Three pre school children need health statement.
It was observed that caregiver did not wash her hands nor the child's hand after diaper changing.
A caregiver did not see a large scratch on child's buttock when changing child's diaper 5 - 10 times a day.
A child was served food that contained tree nuts which child is allergic to. Caregiver stated that they didn't check the ingredient before serve it to child.
Two hours of transportation safety tranining needed for two drivers. Their last training expired in October and December of 2021.
Health statement needed for one pre-school child in care.
Proof of education is needed for one caregiver.
Six inches of loos-fill surfacing is needed on the climbing equipment.
