Annual Fire Inspection was observed to not be conducted since October, 10th 2024.

Five children records evaluated were missing one or more of the following: -Date of Admission; -Address of emergency contact information; -Physician contact information or emergency care facility Note: Correction was made during the inspection.

A caregiver was observed in a classroom and who is counted in child/caregiver ratio does not have at least 8 hours of pre-service training.

A child in the 18m -2yr. old classroom was observed asleep on their nap mat in front of the classroom's exit door. Note: The Director made a correction by moving the child to another area of the classroom during the inspection.

A caregiver in the 18m -2yr. old classroom did not change their gloves between diaper changes. The caregiver changed three children diapers using the same gloves. Note: Correction was made during the inspection.

A caregiver in the 18m -2yr. old classroom did not change the sanitary liners or sanitize the diaper changing pad between diaper changes. The caregiver changed three children diapers before sanitizing the changing pad or using a new sanitary liner. Note: Correction was made during the inspection.

The diaper changing mat in the18m to 2yr. old classroom contained rips/tears in it. Note: The Director replaced the mat with a new one during the inspection.

The operation has a tire swing attached to a composite structure.

The operational policies lacked the complete policy required of the minimum standard. This was corrected at the inspection.
