During the investigation it was determined a child was sent home with a wet diaper and had to be brought back in to be changed.

Groups of children were also observed, during inspection, to be running through the building, from one class area to another without structure of the caregiver being able to see where the children are or what they are doing. The management of children's behaviors are not under control.

The infant room has a wall that needs border put up to cover up torn walls at the base of the floor where paint can be pulled off and put into an infants mouth. There is also a place under the window area that needs repaired or covered. The entrance to the infant room has carpet that is frayed and needs to be covered. The large room has a wall that h as holes and peeling paint on the bottom that needs repairing or covering. The girls restroom has plaster on the wall by the left sink that needs repaired or covered.
