During the course of the investigation, it was determined that a medication was given to a child without written consent from the parent or the doctor.

An infant was moved directly to the group feeding table after having their diaper changed without having their hands washed. This was corrected at inspection when the caregiver was advised to remove the child and wash their hands.
The infant room had six children, five of which were 11 months or under, with one caregiver. This was corrected at inspection when a second caregiver was brought into the room.
Seven children whose ages ranged from 18 to 23 months were observed having screen time. This was corrected at inspection when the caregiver opened up the centers in the room.
An infant was observed sleeping in a bouncer and another was observed sleeping in a swing. This was corrected at inspection when the children were moved to their cribs.
There were two rooms running with nap time ratios and one caregiver in the infant room with no one else was in the building. This was corrected at inspection when a controlling person was called in.
A provisional employee was observed in charge of a group of children by herself.
During the inspection eight children, whose ages ranged from 3 to 17 months, were observed with one caregiver. The specified age group was 0 to 11 months.
Based on the information gathered during the course of the investigation it was determined that minimum standards regarding ratios and supervision were regularly disregarded due to staffing issues. No effort was made to adjust the student population in order to meet minimum standards.
During the inspection two classrooms that were combined for nap were observed in their room without a caregiver present. Another class was left unattended when the caregiver left it to open the front door. This was corrected at inspection when the cook moved to the room and the other teacher returned to the room.
Based on the information gathered during the course of the investigation it was determined that disciplinary techniques used with children have not been positive.
The PreK teacher in classroom 129 stated there were 10 children in care when we verified there were only eight children.
Based on information gathered, ratios have not been met with the mixed age group.
The infant's feeding instructions were not followed on 7/12/2024.
Evidence gathered for this investigation determined the daily report for an infant had incorrect information on the third feeding on time and ounces served to the infant.
The exit gate on the Pre-K playground was open and unsecured.
It is found parents are not being given completed infant daily reports daily.
During an investigation, it was found that a caregiver did not have an eligible background check on file.
One of five employee's files did not contain evidence of a high school diploma or GED.
The infant caregiver did not wash her hands after changing a diaper. She was observed using hand sanitizer when a sink was available.
The operation does not have a current Fire Inspection. The last inspection date is 9.20.2020.

Gaps in the shock absorbing tiles in the two-year-old playground have separated creating gaps large enough to create a trip and fall hazard.
