A child was napping without a mat. Corrected at Inspection: A mat was provided to the child to nap.

A fire drill was not conducted for the month, May 2025.

There was no food allergy emergency plan available for review for a child with a known food allergy that has been diagnosed by a health-care professional.

The director was missing annual training for training year 7/26/2022 - 7/25/2023 on the following topics: (1) Recognizing and preventing shaken baby syndrome and abusive head trauma; and (3) Understanding early childhood brain development. Note: The director was able to complete the training online during the inspection.

A caregiver caring for a group of children under 24 months of age was missing annual teaining for training year 4/12/2023- 4/11/2024 on the following topics: (1) Recognizing and preventing shaken baby syndrome and abusive head trauma; (2) Understanding and using safe sleep practices and preventing sudden infant death syndrome (SIDS); and (3) Understanding early childhood brain development.

Immunization records were not present for 1 out of 6 child files reviewed.

Operational policies on Promotion of indoor and outdoor physical activity were incomplete as observed during the inspection.

The fire inspection was not completed within the 12 month time frame. Last fire inspection was completed in February of 2021.
