Based on the information provided, a child sustained a scratch on the face by a caregiver, when they grabbed the child by the arm and flipped them over onto their cot.
Napping cots in two of the three class rooms observed, were labeled with numbers; there was not a number/child assignment map available. This was corrected when the caregivers in both classes developed a map for their class.
The allergy list was posted in two classes with out a cover. This was corrected when the director turned over the Allergy List to hide the names of the child(ren) names.
An outdated Activity Plan was posted in the Infant room. This was corrected when it was brought to the caregivers attention. She replaced the outdated Activity Plan with the current Activity Plan.
Of the two infants that are still on Formula one did not have current feeding instructions on file. This was corrected when the parent completed the Feeding Schedule.
Changing table in the Toddler room was not clean. This was corrected when the director brought it to the attention of the caregiver and at that point the caregiver sanitized the changing table.

The director was missing the following annual training for 2020-2021: Child Maltreatment, Communicable Diseases including Immunizations, Food Allergies, Building and Premises Safety, and Hazardous Materials including Blood Borne Pathogens.
