During the inspection a caregiver did not know the ages of the children in care when asked. Another caregiver later corrected themselves when stating ages of the children in care.
A 4 year old child in care was observed by herself by an entry door leading into the main building from the playground. While the caregivers were outside, they could not see or hear the 4 year old child in care to safely intervene. This was immediately corrected when the director came to the entry door in close proximity to the child in care.
Both caregivers in the toddler room were unaware how many children were present under their care. There were 16 children ranging from 18 months to 3 years old. The specified age group was 2 years old.
A caregiver was alone in a classroom with 16 children where the specified age group is 2 years old. The ratio for this specified age group is 11 children to 1 caregiver.
A group of children under the age of 2 were observed watching television in the toddler room.
It was observed that the children in the program had not been signed in or out for a period of at least 3 days. This was corrected at inspection when the caregiver signed all of the children in for the day.
Infant feeding instructions were observed to not be updated since August 2021 for infants that were currently not on table food.
It was observed that playground safety checks had not been being documented monthly as required.
All children's nap mats/bedding were observed on a shelf touching one another. This was corrected at inspection when a caregiver placed each nap mat in a separate plastic trash bag and then labled the trash bag with the child's name.
Upon review of employee files, it was discovered that 3 employees were lacking annual trainings hours in required topics.
The operation did not have a current written activity plan nor were there any past activity plans available for review.
