First-aid kit was observed to be missing antiseptic, tweezers, gloves, and a thermometer. Note: corrected at inspection missing items were placed in the first-aid kit.
Caregiver failed to wash hands after diapering 2 children.
Seatbelt in the back passenger side of the van was stuck and could not be used.
2 recalled boppies were seen in the infant room. Note: Children's product certification was observed to be completed and up to date
Toddler room was seen with a peeling baseboard. The bottom of the wired fence has sharp edges poking out and is in reach of the children.

Two classrooms were observed not having a number chart to match the numbers labeled on the cots. NOTE: This was corrected during the inspection with a chart made to match the numbers on the cots.
One caregiver was observed with 9 children and SAG 0-11m.
One medication for a child enrolled was not properly labeled with the child's name and date of entry into the facility. This was corrected during the inspection and withdrawn from medication box and given back to parent.
