Through documentation and the inspection, it was determined that a staff member was present prior to receiving their background check determination.

Staff attendance sheets were not available upon request during investigation inspection on 4/6/2023

It was determined that a communicable disease case at the operation was not reported to licensing within 2 days of its discovery.
