At the time of inspection it was found that the site director had left the program at the end of January in 2026 and this was not reported within the required time frame.

At the time of the inspection the caregiver in charge did not have immediate access to information that allowed them to communicate effectively with parents. She was not provided information about the enrollment status of a child in care which caused confusion about their attendance the day of the inspection.

At the time of the inspection an epi pen present had expired in January of 2026 and was stored with the first aid kit and medicatiosn.

At the time of the inspection the program could not access the cafeteria where the first aid kit was located because the door was locked while the school had a program going on.

At the time inspection the program director had not been on site or in communication with the person left in charge since the site director had left their role in January 2026.

Fire drill practices has not been conducted in the last 4 months.

The active play area and equipment has not been inspected in the last three months.

The last fire drill was documented on 1/23/22.

Last licensing inspection was not posted.

Caregivers were unable to keep children under control, an ISD staff was observed intervening in order to get the children attention.
