During the course of an investigation, it was determined that good judgement was not demonstrated when a caregiver fed an infant breast milk designated for another child.
There was a child left on the playground unsupervised, the child was found by a parent and teachers were notified and then returned the child to the class. The caregiver in charge of the class was terminated.
The surfacing material located around play equipment was damaged revealing a hole about 6 inches wide.
The use zone around the tire swing was overlapping the use zone of another play equipment's use zone. Note: This was corrected at inspection when the play equipment next to the tire swing was moved so the use zones were not overlapping.
Two exit gates on the Pre-K playground had a gap at the bottom of each gate that was larger than three and a half inches. Note: This was corrected at inspection when the gaps on the gates were adjusted.
There was a child enrolled with expired medication on file at the operation. Note: This was corrected at the inspection when the parent arrived and replaced the medication.
