During the course of the investigation, it was determined that caregivers did not ask for or obtain photo identification for a person that picked up a child in care.
There has not been a monthly smoke detector check since January 2022. This was corrected at inspection when the director checked all systems and found them in compliance.
During interviews, it was discovered that a caregiver had been found sleeping when the children were awake and playing in a classroom in June of 2022. A video was obtained that showed this and confirmed that this did occur.
A person with a pending background check was found to be serving in a classroom as a caregiver to children. This was corrected at inspection when the individual left the premises and will not return until an approved background check is received.
Caregivers in two classrooms were not found to be using proper hand-washing proceedures following diaper changes.
An infant younger than 12 months of age was found sleeping in a crib with a blanket. This was corrected at inspection when the caregiver took out the blanket from the crib.
Loose-fill was not properly maintained in an area with a play structure taller than 5 feet. The loose-fill measured between 2-4 inches when it was required to be a minimum of 9 inches in depth. The director agreed that children will not be allowed to play on the structure until the operation is able to bring in more loose-fill to reach the depth as required by minimum standards for child safety.
A caregiver was heard by other staff and admitted to telling a child that they did not like them.
The fire inspection has expired. The last inspection was conducted on 8/31/20. The operation has reached out to the fire department, but they have not come out at this time.
