A fire drill was not documented for the month of February 2023. For January and February 2023 the monthly checks for fire extinguisher checks, smoke detector checks and carbon monoxide detector checks were not documented.

The manufacturer instructions for the test data for the unitary surfacing was not available for review at inspection.

Diaper changes were observed at inspection with infants having their diapers changed but the infants did not have their hands washed afterwards. Additionally, infants did not have their hands washed prior to eating/drinking a bottle. Staff were observed preparing bottles and were not washing their hands prior.
