During the course of the investigation, the director provided false information regarding start dates for newly enrolled children in attendance and attempted to falsify sign in/out sheets.
During a DFPS investigation inspection, the provider was unable to produce an employee's file who left employment on April 17, 2026.
Attendance records available for review were only available for four days in April and eight days in May. The remaining attendance record days for both months were not available for review.
It was determined during the investigation that children are made to cover their heads with their blankets while napping and are not able to remove the blankets.

This person must know they are in charge and know how to respond to situations that could place children at risk. The person in charge must also have access to essential records, which may need to be made accessible to licensing or parents when needed.
