This rule was evaluated with regard to the investigation and a deficiency was found. Based on the information gathered, it was determined that a child was left unsupervised in the playground area for about 1 to 2 minutes.

The operation did not enter the number of employees left in 2023 in a timely manner.

Caregiver did not notify parents after children were left alone in the bathroom with the lights off for an unknown amount of time.

Caregiver did not have self-control while children were present, caregiver yelled at other staff in front of children.

Operation did not notify licensing when leaving two children in the bathroom in the dark unattended for an unknown amount of time.

Caregiver left two children in the bathroom unattended with the lights off for an unknown amount of time.

The director did not have evidence of being routinley present at the operation.

I reviewed infant feeding instructions that were not updated.

The operation unitary surfacing has not been maintained as instructed.

The operation did not have a updated fire inspection available for review. Last inspection was conducted on 3/11/22.

I reviwed 4 personnel files that were missing, affidavits, Background check letters, HS diploma, First Aid/CPR.

Cribs were observed with mattresses that do not fit the crib properly.

The operation does not have documentation for the cribs. All documentation on the cribs have been removed.
