A fire insepction had not been completed for the previous 12 month period.
During the inspection a water hose was observed out in the playground area while children were present.
During the inspection the loose fill was observed to be at less than 1 inch in depth.
During the inspection the walls in one of the bathrooms was observed to have bubbled and peeled from water damage. Some drywall was also exposed in the activity room, and the baseboard was damaged/chipping in the music and computer rooms.
During the inspection when observing the playground the bare ground could be seen around the play structure. The loose fill had also packed down around the structure in multiple spots.
During the investigation it was determined that the daycare did not report an incident where a child received medical attention in the required time frame to CCR.
