In two classrooms, the under-the-sink cabinet had broken child locks which contained cleaning supplies accessible to children. This was corrected when the cleaning supplies were removed from the cabinet and placed in an inaccessible area.
Based on investigation findings, proper supervison was not maintained when a child was left alone and for approx 5 to 8 minutes in the classroom.
A child was left alone in a room for approximately 2 hours.
Based on information obtained from the operation, there is sufficient evidence to support the allegation of a child being left outdoors on the playground.
