During the investigation, interviews and other evidence collected determined that the operation was found deficient with this Minimum Standard. The evidence established that, although the parent notified the operation that the child required transportation from school, the child was not included on the transportation list and was not picked up as scheduled. As a result, the operation failed to maintain and utilize an accurate list of children to be transported, resulting in the child being left at school.
