A caregiver hired on August 4, 2026 was counted in ratio in the toddler room without completing 8 hours of preservice training in required topics.

3 caregiver files reviewed lacked 24 hours of annual training for the previous completed training year.

It was determined through video surveillance that a caregiver lacked good judgment when they picked up a child in care by one arm and inappropriately carried the child down the hall.

A caregvier from the toddler room was observed out of the classroom at the front desk leaving their co-caregiver in the classroom out of ratio with 10 children where the age group of children was 10 months to 18months.

A substitute caregiver in the 2 yr old room that was hired in April 2024 did not have current First aid and CPR.

It was determined through the course of the investigation that the operation violated minimum standards by leaving 2 caregivers, who were within their 90 days of employment and lacked training in pediatric first aid and rescue breathing, unsupervised with children during naptime.

A deep hole was observed in the ground of the outdoor play space adjacent to the sidewalk causing a hazard to children in care.

A 15 month old infant was observed unbuckled in a highchair. This was corrected at inspection when the child was buckled into the highchair.
