Children were observed at snack time with their animal crackers placed directly on the table for snack time instead of being placed on a napkin or plate.

One child's food allergy plan was lacking a physician signature.

Children were placed at the table for snack time without having their hands washed.

An infant that is less than 12 months year of age was observed at inspection asleep in the crib and covered with a blanket. The infant and blanket was removed from the crib at inspection.

The most recent annual fire inspection was 2/4/2022 and was conducted more than 12 months ago.

Four admission records reviewed were lacking a current immunization record.
