During an unannounced inspection, a television was observed in a mixed-age classroom that includes children under the age of 2 years, which was confirmed to be used at least once per week on Fridays. Any use of screen time is prohibited with children under 2 years of age. The Director removed the television from the classroom, so this was corrected during the inspection.
During the unannounced inspection, a television monitor was observed on topp of a shelf in the preschool classroom and was not mounted to secure it and prevent it from tipping over. The Director removed the television from the classroom, so this was corrected during the inspection.
During an unannounced inspection, the electrical outlet in the preschool classroom was observed to have the cover plate removed, exposing the area around and behind the outlet. This outlet is on the lower part of the wall and is easily accessible to the children.

During the course of the investigation, it was determined that a caregiver used prohibited punishment when she hit a child with a toy to demonstrate how it hurt and threw a ball at the child.

During the inspection, it was observed, and a photo was taken, that backpacks were piled on the floor in between two shelving units and not kept separate.
During the inspection, it was determined that the recent copy of the emergency drills being practiced was not up to date.
A stand alone slide was observed having a pole in the use space zone that was less than six feet.
