Two caregivers did not have documentation that they received personnel policies.

The operation had a water holes on the pre-school and school age playground. They was a broken toy container on the school age playground. The operation had a deep area on the school age playground to where the ground was not level and could cause a tripping hazard.

Four children did not have documentation on the parents receiving the operational polices.

The operation did not have the required surfacing under and around the merry go around on the pre-school playground.

The main first aid kid did not have a first aid guide.

Two children did not have a parent signature on the food allergy plan.

Four caregivers did not have documentation that they received center policies.

Two children did not have they Epi-Pens in the original container they were placed in a zip lock bag.

One caregiver did not have a Pre-Employment Affidavit in the file, start date 1/1/2025.
