At the time of inspection, the background check has not been submitted for a caregiver counted in ratio.

For a critical injury, the operation failed to contact EMS first.

When reviewing records for an investigation, it was found that one caregiver does not have documentation showing their education is equivalent to a High School diploma and their education is in spanish.

Toddler's sippy cups and bottles were not all labeled in two classrooms.
